Application form for refunds following delay

I was wondering if anyone here knows where the application form for applying for a refund (following a delay) can be found on the new Irish Rail website?

If it was a reserved ticket, cover letter plus ticket to

Dublin 1

Given reason as delay, train cancelled or left standing despite reservation. Refund in case to the credit/debit card used.
