Default VIA Rail Canada fatal derailment

A rebuilt F40PH is on its side tonight sadly and three VIA Rail employees dead after a derailment at Burlington west of Toronto. Speeds in the area would have been something around 75mph. About 75 passengers on board coming from Niagara Falls.

latest info is that the train took a 15mph switch at 67mph. Loco 6444 was fitted with an event recorder but not a voice recorder so it is not known who was the driver and what signals were called (VIA seems to operate with 2 in the cab with a call/acknowledge system between driver and accompanying engineer when signals are sighted). The Transportation Safety Board are now making an issue about a failure to take action on a 2003 recommendation to fit voice recorders since it would be clearer which engineer was in charge at the time and whether the signals called matched the switch position.

In regards to the 22000 class, being the newest - are they fitted with data-only recorders or are they also fitted with a voice recorder / forward looking video? (Similar to the "Silent Witness" on Amtrak Acelas and other types.

The 22000s are fitted with forward-looking video. There has been some very good still shots in recent RIAU reports into level-crossing accidents: the cameras seem to have captured everything.

I would be surprised if there was a cab voice recorder in the 22000. It would be of limited value as the cab is normally single manned and any conversations with CTC on radio or via line-side telephone would be recorded in any evemt. There is no procedure for "calling" signals as in North American practice other than in the event of the failure of on-board CAWS equipment where a secondman must be carried.

I imagine the forward looking camera deals with the vast bulk of such situations in Ireland.

The ICR camera system records forward/rear and sides

All radio/line side phones are recorded centrally. The signaling records are also stored

The on train data recorder, records the cab signaling state and all control inputs made. The ICR's also are being equipped to phone home and to download diagnostic information to base. Following an accident there is a good chance the onboard computer is still intact and there a huge amount of data in there

sounds a lot better than what VIA had fitted - TSB had to find "a compatible locomotive" (presumably another F40) to download the data from rather than a standalone diagnostic unit.
