Must be a big desk, they managed to lose my last refund form as well, that said I had a photocopy for that contingency

Note to confuse matters stuff for Heuston based services bar Waterford should go to customer relations southern and western Heuston (703 4499), the forms in Heuston have that address the ones in Thurles and elsewhere don't. Thurles is the only station where the forms are actually on display
