Recently have spotted postcards available from a box in some stations "Customers...please tell us your concerns Help us to Keep Your Station Safe".

The cards can be removed from the box, filled in and then deposited in the post box provided.

The reverse of the card reads:
"Customer Safety Report Form

Your safety is our number one priority. If you see anything at this station which is a safety concern, please detail your particular concern below and place your completed card into the post box provided. Your concerns will be addressed by the Station Manager.
